Young man presenting gait ataxia and vertigo since last year.
Microsurgery perfomed on telovelar approach via suboccipital craniotomy. A cysticercal cyst fulfills the fourth ventricle with slight brainstem compression.
Intraoperatively, observed arachnoiditis of tela choroidea and thickness of meddullary inferior velum as well. Due chronicity of infection, the whole cyst were adhered on ventricle walls, performing the dissecciont unviable. Also observed the cyst projecting through tonsils.
A third ventriculostomy were performed just before the surgery
